The "Activity Stream" provides up-to-the-minute streaming updates and keeps all your conversations in one place. You can directly add comments to work items you are following and /or have view rights to from this report directly.
The default report will show you entries for the entity (project, task, documents, discussions, app items) creation, updation and which you are following. You can edit report and change the filter to suit your requirements.
